Your personal data and your privacy in our general practicek

At our general practice, we handle your personal and medical data with care. In this privacy policy, we explain what data we process, why we do that, how we protect them and what rights you have. We act in accordance with:

  • the General Data Protection Regulation (GDPR)
  • the Medical Treatment Agreement Act (WGBO)

What personal data do we process?

In order to provide you with the best possible treatment and care, we process personal data such as:

  • your name, address, and contact details
  • your date of birth and national insurance number
  • information about your health and treatment
  • information required for claiming reimbursement from your health insurer

Why do we process your data?

We use your personal data exclusively for:

  • good medical care and treatment
  • administration and financial processing
  • practice management and quality improvement
  • scientific research, education and information (with data being anonymised as much as possible)

Your details will be not used for other purposes without your consent.

Use of cookies on our website

Our responsibilities as a general practice

Amstelveen Noord General Practice is responsible for the processing of your personal data. We ensure that:

  • you have been informed about the use of your data (via our healthcare providers, this website or leaflets);
  • all our employees a professional secrecy handle your data confidentially;
  • your details well secured are protected against loss and unauthorised access;
  • your details not be kept longer than necessary.

Retention period

Medical data is stored in accordance with the law. 15 years kept since the last treatment. In some cases, longer storage may be necessary, for example in the interests of your health or that of your children.

Your rights as a patient

You have the right to:

  • to know whether and which personal data will be processed;
  • your details to be seen and of this a copy to receive;
  • to have your details correct or supplement if they are incorrect or incomplete;
  • to disclose (part of) your medical data destroy, unless we are required to retain them by law or if this is detrimental to others;
  • one self-declaration add to your medical file;
  • in certain situations to object against the use of your data.

How can you exercise your rights?

Would you like to exercise one of these rights? If so, you can do so by inform the GP practice verbally. You will receive a form for this purpose. You may also be represented, for example by:

  • a person authorised in writing
  • a curator or guardian
